My husband and his X-wife have agreed that she would stop all child support and he would sign off rights to their child. Where can we find the form online for them to fill out and file with the courts? The locatin is Fort Wayne, Indiana (Allen County) I am not going to go through 10 years of reasons why they have decided to do this but many counslers have agreed that he needs to protect his children that he lives with and does have a relationship with vs. one who don't want a relationship with him and has done harmful things to the other children. He has had the same job for 10+yrs. and has paid $260 a month for the past 9 1/2 yrs. him being able to pay is not the matter at hand it is just a way to cut ALL ties once and for all. I don't want to be rude all I am asking for is information on the forms no questions or criticism. Thank you for your time You would not believe the hell my husband has been put through for the past 10 years...CPS, Police, Court hearings. His x-wife has substantiated lack of supervision twice, 1 medical neglect, 1 neglect and the courts keep allowing her to have custody of their daughter. His daughter has learned everything that a noraml sound and stable parent dreads but mother sees no problem with her life style and daughter wants and is following in her mothers footsteps and yes he did take her to court for custody and spent over $10,000 and had 33 police reports and CPS reports and witnesses and he did not meet the burdon of the courts to have her removed.
